Schedule Your Appointments:

Scheduling your appointments on line is quick and easy. Just click on the “Book Your Appointment” icon below and follow the prompts.

If you are a new client please click on the appropriate tab to download, print and fill out forms before coming in for your appointment. This will speed up the intake process and get you on the table faster so no time is lost from your appointment.

Method of Payment

I accept the following forms of payment:

  • No Fault
  • Cash
  • Check
  • Venmo, Zelle (use my phone # 716-342-4009 or user name Cindy-Hebert-11)
  • Credit  Card (visa, master card, Discover)
  • FSA, HSA (I do not accept Independent Health)

You will be asked to provide credit card information to reserve your spot on my calendar. Please note, Your card will not be charged at that time.

Cancellation and No Show

  • 24 hours notice required to avoid cancellation fee
  • you will be charged the full price of the service scheduled if you fail to contact me of a cancellation

 I understand that situations arise in which you must cancel your appointment. If you are unable to keep your scheduled appointment, you must give 24 hours notice. Doing so will give me time to fill the slot with someone who may be on my waiting list, or with someone who may have an emergency and needs to be seen that day. For any late cancellations or no shows you will be charged the full amount of the service you were scheduled for. Charges that are incurred will be billed to your credit card on file or paid at the time of your next visit.

You will not be charged if you are sick, an emergency arises or for bad weather.
